Senior care by city, ranked on the public record
We build a ranking page for every U.S. city with at least 5 nursing homes — ordered strictly by the government (CMS) star rating, with the independent health-inspection score as the tie-break, and no pay-for-placement. Where a whole state has no city that big, we rank the state itself, so every state is covered. For assisted living (cities with 6+ communities) there is no government rating, so we list every licensed community instead of pretending to rank them. 1,958 city and state pages in all — pick a state.
Ranking is only a starting point — and the star we sort on is a blend. CMS builds its overall rating from inspections, staffing and quality measures, and the last two lean on what facilities report about themselves, so an overall star can sit above what inspectors actually found. We sort by it anyway because it is the government's rating and not one we invented, and then we un-blend it: every hub puts the independent inspection star, the citations and the fines on the row. A top-ranked home can still carry a serious citation or a fine, and a modestly-ranked one can have a spotless inspection record — compare the record, not just the star.
